TARGETED CASE MANAGEMENT program assists parents in finding and setting up appropriate services for children with special needs. Case Managers work as advocates for the child and family’s needs in a variety of settings. All services are approached with the best interest of the child and family in mind, and with a high level of respect and dignity through a team-based approach. Areas of focus include:
- identifying community services in the area of the child’s home
- coordination of current and desired services
- advocacy for the child’s rights
- monitoring of services provided
- referring for any necessary supportive or therapeutic services
- supporting parents to overcome individual barriers they may face
Children who are in need of funding outside the family resources must have a mental health, chronic illness or development delay diagnosis to qualify for this program.
