Children’s Case Management

TARGETED CASE MANAGEMENT program assists parents in finding and setting up appropriate services for children with special needs. Case Managers work as advocates for the child and family’s needs in a variety of settings. All services are approached with the best interest of the child and family in mind, and with a high level of respect and dignity through a team-based approach. Areas of focus include:

  • identifying community services in the area of the child’s home
  • coordination of current and desired services
  • advocacy for the child’s rights
  • monitoring of services provided
  • referring for any necessary supportive or therapeutic services
  • supporting parents to overcome individual barriers they may face

Children who are in need of funding outside the family resources must have a mental health, chronic illness or development delay diagnosis to qualify for this program.
